Permission, Consent, and Release Form for 2023-2024 Term Consent Form and Program Information I am aware and give consent for DramaWay to take photographs and video, including online sessions during the designated time of the program, including online and in-person sessions. Release Form for Online & In-Person DramaWay and/or DramaWay Productions Programs: I accept my responsibility in providing a safe place for movement and activity for the above-named participant during DramaWay and/or DramaWay Productions’ virtual sessions. I understand that I remain solely responsible for the participants' care and medical needs during all DramaWay and/or DramaWay Productions online sessions. I also accept responsibility for this participant’s own medical coverage while attending any In-Person programs arranged by DramaWay and/or DramaWay Productions. If attending In-Person DramaWay and/or DramaWay Productions sessions, I hereby give permission to Facilitators/Consultants & Volunteers to arrange for any emergency medical care including hospitalization and transportation if necessary and agree to pay for all expenses and costs incurred thereby. If emergency medical care is required while attending In-Person sessions, attempts will be made to contact the emergency contact person(s) shown above. I agree to release and indemnify and save harmless DramaWay and DramaWay Productions, c.o.b. Danielle T. Strnad and their Facilitators/Consultants as well as Volunteers from all claims arising from whatever participation in any program organized by DramaWay and/or DramaWay Productions by any cause whatsoever. I acknowledge this form with its legal consent and liabilities remain in effect from the date it is signed and will apply to any future programs that the above-named participant is registered in unless advised otherwise in writing. I, the undersigned, have read, and fully understand the “Participant’s Release Form"

Refund Policy: If a course is canceled due to low enrollment, registrants will be given one week's notice and refunds will be issued subsequently. No refunds will be given after the first class. A full commitment to attendance and participation is encouraged. We regret that reimbursement for missed classes cannot be offered. Please note that DramaWay and/or Dramaway Productions reserves the right to remove a participant from the program if we deem it appropriate. However, every measure will be taken to work with the participant and his/her family before this severe action is taken
